Jessica Van Fleet-Green, MD, CMD, FAAFP

Dr. Van Fleet-Green is a Family Physician, a Fellow of the American Academy of Family Physicians, and a Certified Medical Director for Post-Acute and Long-Term Care. She has over ten years of medical leadership experience in various roles including post-acute medical directorship, executive leadership in an Independent Physician Association, and consulting roles locally and nationally on hospital transitions & post-acute. She is now a physician executive with MultiCare Health System and in early 2022, was the clinical leader responsible for launching the organization’s first PACE program. 

Dr. Van Fleet-Green practices medicine using a patient-centric, whole-person approach and is well-versed in leveraging an interdisciplinary team and the Institute for Healthcare Improvement’s Age-Friendly Health System framework. She has extensive experience with the complex care of patients 55 and older. She has represented Long-Term Care on the Washington Disaster Medical Advisory Committee and is active in policy and advocacy as a board member for the Washington Society of Post-Acute and Long-Term Care. She teaches Family Medicine residents through the University of Washington Department of Family Medicine and is a teacher for Serious Illness Conversations.
