PGY2 Ambulatory Care Pharmacy residency

About the program

MultiCare Ambulatory Care Pharmacy services are multifaceted and continue to expand. Our team provides care at clinic locations in and around Puyallup and Tacoma, Wash. Ambulatory care clinical pharmacists practice in primary care and specialty settings, providing direct patient care under collaborative drug therapy agreements as credentialed providers.

Pharmacy residents are an integral part of MultiCare’s family medicine residency programs, working with the two faculty pharmacists longitudinally. Rotations are available in primary care, diabetes management/education, hospice/palliative care, neurology, cardiology, psychiatry, outpatient oncology and others based on resident interest. The program emphasizes chronic disease management and precepting and teaching skills. The position is funded by a grant for rural medical education and includes a four-week rotation to provide ambulatory pharmacy services and medical education in Omak, Wash., as part of a new family medicine residency training program.

Curriculum

The residency program will develop competent clinical practitioners who are ready to provide evidence-based, patient-centered care to a diverse patient population. The curriculum prepares graduates to be skillful clinicians in a variety of settings.

Residents may collaborate with preceptors to develop new elective experiences based on their interests and goals.

Staffing commitment

Residents teach group diabetes class in the evening twice per month, as well as staffing two days in the medication management clinics per month.

Meet our team

Residency program director: Leanna Davis

Leanna Davis is a board-certified ambulatory care pharmacist and diabetes care and education specialist. In her role as faculty pharmacist for Tacoma Family Medicine residency, she provides didactic and experiential education for medical and pharmacy residents as well as maintaining her own patient panel.

Residency program coordinator: Christy Weiland

Christy Weiland is the faculty pharmacist for East Pierce Family Medicine residency and is a board-certified pharmacotherapy specialist. She practices both inpatient with the medical residency team and as an ambulatory care pharmacist, and provides medical education to medical and pharmacy residents.

Current resident

Pharmacy resident Lynn LuuLynn Luu graduated from Washington State University School of Pharmacy and completed her PGY1 training at Veterans Affairs Puget Sound Health Care System in Seattle, Washington. Her career interests include primary care, diabetes management, and oncology. Her residency project is creating a collaborative visit model for diabetes care at a primary care clinic. She is originally from Honolulu, Hawaii and enjoys reading, being outdoors, and trying new foods especially spicy foods.

Preceptors

Jalena Bown is the primary preceptor for the hospice/palliative care rotation. She is the clinical pharmacist for MultiCare’s hospice team, helping patients with medication needs as they approach the end of life. She collaborates with an interdisciplinary team to provide comfort and education regarding the many symptoms and complications of care on hospice.

Brittany Flesher is the preceptor for the primary care medical home rotation and a board-certified ambulatory care pharmacist. In her practice at Frederickson Primary Care Clinic, she sees patients for many chronic diseases and educational needs.

Brilliana Hou is the preceptor for the neurology rotation. During this rotation, residents expand on knowledge of neurologic disease states and work with neurologists to manage medications for multiple sclerosis, migraines, epilepsy, movement disorders, cognitive disorders and more. Hou is originally from Pullman, Wash., and received her bachelor’s degree and PharmD from University of Washington. She then completed a PGY1 at University of Washington – Valley Medical Center and an ambulatory care PGY2 at The Ohio State University Wexner Medical Center. When not working, she likes to go hiking, watch TV (especially reality shows) and try to get her dog to like her.

Michael Lee is the preceptor for the managed care pharmacy rotation. He performs utilization and cost trend analysis, develops clinical programs and quality improvement initiatives, and facilitates care coordination for populations under risk-based arrange contracts MultiCare has with various payors. In addition, he participates in P&T and other pharmacy strategic meetings to help promote more value-based care for the system.

Julianne Ramirez-Nadjm is the preceptor for the diabetes management and education rotation. She is a certified diabetes care and education specialist and is board certified in ambulatory care, pharmacotherapy, advanced diabetes management and geriatric pharmacy. She provides education, medication management and diabetes technology expertise to patients with all types of diabetes.

Jeannette Quimby is the primary preceptor for the medication management clinic (MMC) rotation. She is the lead pharmacist at the Gig Harbor MMC, where she assists patients in achieving improved outcomes through anticoagulation management, diabetes medication management and smoking cessation. She is also responsible for clinic compliance, quality and productivity outcomes. With a background in communications, she loves direct patient contact and working with residents.

Living in the South Puget Sound

The South Puget Sound has something for everyone. Each community boasts its own personality and amenities. Some of our favorite things about Tacoma and Puyallup are:

  • Easy outdoor access: Within a two-hour drive of Mount Rainer National Park, Olympic National Park, Pacific Ocean beaches and skiing at Crystal Mountain or Snoqualmie Pass
  • First-class museums including the Museum of Glass, LeMay-America’s Car Museum, Tacoma Art Museum and the Washington State History Museum
  • Cultural opportunities with local theater, symphony orchestra and band and opera companies
  • Diverse community that prides itself on its grit

Housing is provided for the required four-week rural medicine rotation in Omak, Wash.

Eligible candidates will be a graduate (prior or anticipated) of an ACPE-accredited college of pharmacy or Foreign Pharmacy Graduate Equivalency Committee (FPGEC) certificate, and is licensed or eligible for licensure in Washington state. They must satisfy eligibility requirements for employment, including acceptable results on a pre-employment drug screen and background check. Please note that MultiCare does not sponsor work visas.
